Kahoot is very engaging...limited to multiple choice questions.
With plickers you don't even need any devices for students, just the individualized free plicker cards for multiple choice responses. The teacher dowloads the app.

Excel Survey in Office 365 can be used to collect information for exit slips. It even has a quick way to create a "QR" code directly to the document.
